If you are a patient: the personal data we will process to facilitate a booking usually includes detailed personal information such as; name, age, sex, date of birth, contact details (telephone numbers and email addresses), address, time of booking, date and location of booking and other relevant text relating to the booking.
If you are a healthcare provider: the personal data we will handle includes but is not limited to; age, sex, date of birth, contact details (telephone numbers and email addresses), work places, qualifications, languages spoken and/or calendar.
We also collect content or other information that you may provide or create when you interact with our Services.

By providing Your personal information to us You consent to us storing Your personal information on servers hosted in the EU. While Your personal information will be stored on servers located in the EU, it will remain within our effective control at all times. The server host’s role is limited to providing a hosting and storage service to Checkup, and we’ve taken steps to ensure that our server hosts do not have access to, and use the necessary level of protection for, Your personal information.

You have the right to ask us not to process your personal data for marketing purposes. We will usually inform you (before collecting your data) if we intend to use your data for such purposes or if we intend to disclose your information to any third party for such purposes.
